8. Criminal Conviction Declaration

Open Academy London is keen to support learners with criminal convictions, and a declaration will not necessarily prevent you from taking up a place. Entry is dependent on you declaring all unspent convictions (and spent convictions where applicable under the Rehabilitation of Offenders Act 1974), as this course relates to work in a healthcare setting. Any disclosure will be reviewed confidentially by our Safeguarding Team.

9. Terms & Conditions
  1. The course fee for your selected package(s) will be confirmed on your invoice. Full payment is required at the time of course booking; enrolment is confirmed only upon receipt of payment.
  2. Learners arriving up to 30 minutes after the start of a teaching session will be permitted to enter the class. Learners expecting to arrive later than this must inform us promptly, and will be required to arrange to recover the missed content at a future class or with their peers.
  3. Full attendance and completion of all components of your selected package(s) is required. Failure to complete the full duration will result in non-completion of the course, and certificates will not be issued.
  4. All live bleeding sessions must be pre-booked and are subject to availability. Cancellation of a live bleeding session requires a minimum of 72 hours' notice; a late cancellation fee may apply and must be paid before any future session is scheduled.
  5. Live bleeding session dates are provided after successful completion of the competency (theory) component. Live bleeding sessions must commence soon after the completion of the competency course; learners who delay starting their sessions may be required to retake the full course at the full course fee. Learners are responsible for ensuring they have received the Hepatitis B vaccination before attending live bleeding sessions.
  6. The Online Self-Paced Training (Package D, 14 hours) is delivered fully online and does not include live bleeding sessions. Learners wishing to undertake live bleeding must also select an advanced training option (Package B or C).
  7. All learners must adhere to Personal Protective Equipment (PPE) guidelines at all times.
  8. Where fewer than 8 learners are registered for a classroom course, or where delivery is not feasible, learners will be informed and course dates rescheduled. Live bleeding sessions may be cancelled or terminated at OAL's discretion to ensure the safety of staff and the integrity of the sessions; OAL shall not be liable for refunds or incurred costs in such cases. Learners are directly supervised during live bleeding sessions.
  9. A certificate will be issued within 7 working days of successful completion of your selected course components.
  10. Should you cancel or withdraw from the course after 14 days from your enrolment date, all outstanding course fees remain due and payable in accordance with our refund policy.
  11. You agree to comply with all Open Academy London policies, rules, and regulations as set out in the Learner Handbook and at www.openacademylondon.co.uk. These Terms and Conditions may be updated from time to time; amendments will be posted on the website.
10. Privacy Notice

In order to process your application, we need to store and process your personal data. Your information will only be used to administer your enrolment and deliver your training, and will not be shared with third parties without your consent, except where required by law. 11. Learner Declaration

I have carefully read this form, including the Terms & Conditions above, and fully understand its purpose and effect. I confirm that the information provided is accurate and complete. I understand the attendance and booking requirements set out above, that I am responsible for obtaining the Hepatitis B vaccination before attending live bleeding sessions, and that the fee for my selected package(s) will be confirmed on my invoice. I have voluntarily signed this agreement of my own free will.
